Learn How to Better Manage Diabetes with "Live in Control"

NCK Medical Center is partnering with Kansas State University Extension to offer Live in Control, a free, four-week diabetes self-management education program designed to help individuals living with diabetes gain confidence, knowledge, and practical tools to better manage their health.
The program focuses on the seven pillars of diabetes self-care, providing participants with education and support in areas such as healthy eating, physical activity, monitoring blood sugar, medication management, problem solving, reducing risks, and healthy coping strategies. Whether you are newly diagnosed or have been managing diabetes for years, this program offers valuable information to support your day-to-day care.
Care supporters, including family members or caregivers, are strongly encouraged to attend. Having a support system can make a meaningful difference in managing diabetes successfully.

- Dates: Thursdays – January 15, 22, 29, and February 5
- Time: 2:00–4:00 p.m.
- Location: NCK Medical Center, 155 College Drive, Concordia, KS 66901
- Cost: FREE
- Total Education Time: 8 hours
